Spending time working with our current users of Discharge to Assess, we have shaped the new digital module for the 4 pathways of IC. We’ve identified that there can be over one hundred cases active within IC at any one time in a Local Authority and each LA may work with multiple NHS Trusts. Some may have two or more hospitals/sites alongside a number of multi-disciplinary teams, community hospitals, care homes and care providers.
With clear dashboards and reporting, teams can see the people supported in IC by each specific service, all in one place.
Intermediate Care provides a solution for relevant Intermediate care partners – from domiciliary care agencies, district nurses, to OTs, assessors, and managers access to the ‘single source of truth,’ it will extend the value already being realised by D2A partners, as well as extend improved management information and metrics to intermediate care. For elderly patients, receiving their intermediate care packages in a timely and supportive manor is essential for their recovery and not being readmitted into hospital.
